The presence of intrathecal IgG synthesis suggests an inflammatory disease of the central nervous system, such as multiple sclerosis (MS).

The HYDRAGEL CSF ISOFOCUSING kit, used on the HYDRASYS 2 SCAN FOCUSING instrument, enables qualitative detection and immunological characterization of oligoclonal IgG bands in human cerebrospinal fluid (CSF) and serum.
The HYDRASYS semi-automated instrument performs migration, immunofixation, and direct detection on agarose gels to generate IgG profiles for qualitative analysis.

Key recommendations from the European consensus report for the diagnosis of multiple sclerosis include:

  • Isoelectric focusing (IEF) on agarose gel for the separation of immunoglobulins

  • Use of non-concentrated CSF material

  • Simultaneous analysis of patient serum and CSF

  • Identical IgG concentration in paired serum and CSF samples

The presence of oligoclonal bands exclusively in the CSF, indicating local IgG synthesis, is strongly correlated with the diagnosis of multiple sclerosis. Their presence provides valuable information when consistent with other clinical investigations.

The HYDRAGEL CSF ISOFOCUSING test is based on the principle of isoelectric focusing on agarose gel, followed by immunofixation. The patient’s serum and CSF samples are analyzed in parallel to compare the distribution of IgG.

Before performing the test, it is essential to quantify IgG in both fluids in order to adjust the IgG concentration to the same level in CSF and serum.
After separation of proteins based on their isoelectric point, the gel is directly incubated with a peroxidase-labeled anti-IgG antiserum, without the need for membrane transfer.
The gel is then treated to remove unbound proteins before the enzymatic reaction.
Finally, the gel is visually evaluated.
